The Practitioner Orders for Life-Sustaining Treatment (POLST) Paradigm Program is designed to improve quality of care for those who are seriously ill by ensuring that their treatment wishes are respected and clearly communicated to their healthcare team at the end of life. Barbara Vaning, MHA, EMT Training Center Coordinator, Penn Medicine Princeton Health Community Wellness, will explain both the POLST form and Advance Directives, highlighting the important differences between the two.
